National IDs and Civil Registration
How do I obtain a Zimbabwe death certificate?
Step-by-step Zimbabwe death registration guidance, from obtaining a burial order to applying for the death certificate with the right informant.

Obtaining a Zimbabwe death certificate usually involves two connected steps: securing authority for burial and registering the death so that the certificate can be issued. The Civil Registry Department says a death should be registered within 30 days. Begin promptly, because hospital, police, burial and family evidence is easier to assemble while events are recent.
First obtain the burial order
A near relative should attend with their national ID and the deceased person’s ID, passport or birth certificate. Take the BD12 medical certificate stating the cause of death. Where a post-mortem applies, take Form 231. The Department publishes the burial order as free with a one-day service period.

Do not confuse the burial order with the death certificate. The order authorises burial; the certificate is the enduring civil record needed for estates, pensions, insurance, property, remarriage and other legal processes.
Identify the proper applicant
The Department lists the people entitled to receive the death certificate in this order: surviving spouse, children, parents, then a close relative. The person applying should carry their ID and evidence that explains the relationship where it may not be obvious. A representative dealing with an estate may need a nomination affidavit, executor documentation or guardianship evidence.
Register and request the certificate
- Gather the burial order, your ID and the deceased’s available identity records. Add the marriage certificate where the applicant is a spouse.
- If there is no burial order, arrange one informant and two competent witnesses. They should carry IDs, must have been present, and must have been at least 18 at the time of death. Also obtain the local-authority letter and hospital or clinic records requested for the case.
- Take documents proving marital circumstances where relevant: a registered marriage certificate, customary-marriage affidavits, a divorce order or evidence that a former spouse was widowed.
- Attend the Civil Registry office, provide the facts of the death and review the entry closely. Check full names, ID number, date, place, marital status and cause information.
- Pay only through the official channel. The Department publishes a US\$2 death-certificate fee and a one-day service period. Confirm current arrangements and retain the receipt.
If the death occurred in unusual circumstances
A police investigation, post-mortem, unidentified body, death abroad or missing medical cause can change the evidence required. Do not guess the cause or ask a witness to sign facts they do not know. Contact the registry and the responsible medical or police authority for case-specific instructions.
There is no fully online substitute for original medical documents, the informant and witness checks. Online contact can confirm the correct office and documents. When the certificate is issued, inspect it before leaving. An error in a name, date or marital status can obstruct estate administration long after the funeral, so accuracy is as important as speed. Make certified copies only after checking the issued certificate. Give copies, not the sole original, to banks and insurers whenever their rules permit. Label the secure storage envelope with the official entry details.

Frequently asked questions
What documents and requirements are needed for a death certificate in Zimbabwe?
The documents needed for a Zimbabwe death certificate depend largely on whether a burial order already exists and who is applying. The Civil Registry Department gives priority to the surviving spouse, followed by the deceased’s children, parents and then a close relative. Start by establishing the applicant’s relationship and collecting the records created at the time of death.

Zimbabwe death registration is handled by the Civil Registry Department at Central Registry and its provincial, district and sub-office network. The Department specifically states that death-registration services are available through Central Registry, provincial and district offices and sub-offices. In most cases, begin with the office serving the place of death or burial and confirm that it can issue the certificate.
Read the full guideWhat can delay or cause rejection of a death certificate application in Zimbabwe?
Death-certificate delays are especially difficult because families often need the document for an estate, pension, insurance claim or property transfer. Most delays arise when the death itself is not fully documented, the person applying has not proved their relationship or authority, or the facts supplied by medical, family and local records do not match.
