National IDs and Civil Registration
What documents and requirements are needed for a death certificate in Zimbabwe?
A clear Zimbabwe death-certificate checklist covering burial orders, IDs, witnesses, marital evidence and estate representatives.

The documents needed for a Zimbabwe death certificate depend largely on whether a burial order already exists and who is applying. The Civil Registry Department gives priority to the surviving spouse, followed by the deceased’s children, parents and then a close relative. Start by establishing the applicant’s relationship and collecting the records created at the time of death.
Standard file when there is a burial order
Prepare:
- The original burial order.
- The applicant’s national ID.
- The deceased’s ID, passport or birth certificate, where available.
- The marriage certificate if a surviving spouse is applying.
- Any official document the registrar requests to connect the applicant to the deceased.
The burial order is normally obtained by a near relative using their ID, the deceased’s identity document, the BD12 medical certificate of cause of death and Form 231 where a post-mortem applies. Keep copies of these records even after the order is issued.

If there is no burial order
The published checklist calls for one informant and two competent witnesses with their IDs. They must have been present and at least 18 years old at the time of death. The file should also include a letter from the local authority and hospital or clinic records. Confirm the precise wording and issuing office for the local-authority letter before arranging the visit.
Witnesses must be people with genuine knowledge, not convenient substitutes. Their accounts should agree on identity, date and place. If the death involved police or a post-mortem, take the official documentation generated by that process.

Marital and family-status evidence
Take a registered marriage certificate when applicable. For a customary marriage, the Department lists affidavits. Divorce or prior widowhood may also need proof so the registrar can record the correct marital status and identify the appropriate spouse. Use truthful documents; a death-registration application should not be used to settle a disputed marriage informally.
An applicant acting for another relative or an estate may need a nomination affidavit, proof of appointment as executor or guardianship documents. Ask the office which authority matches the task. An executor’s role in estate administration does not erase the registry’s rules about who may receive the certificate.
Administrative requirements
The death should be registered within 30 days. Take originals and clear copies, check that names and ID numbers agree, and ensure every attendee carries identification. The Department publishes a US\$2 certificate fee and a one-day service period, while the burial order is listed as free. Confirm current charges, payment methods and office hours.
Before signing, read the entry line by line: deceased’s full name, sex, ID number, date and place of death, marital status, spouse details and cause information. If a fact is uncertain, say so rather than guessing. Accurate evidence protects the family when the certificate is later used for an estate, pension, insurance claim, property transfer or remarriage. Store the burial order, medical certificate, receipt and a certificate copy together. An executor may need that chain when answering later questions about the entry. Record who receives every certified copy.

Frequently asked questions
How do I obtain a Zimbabwe death certificate?
Obtaining a Zimbabwe death certificate usually involves two connected steps: securing authority for burial and registering the death so that the certificate can be issued. The Civil Registry Department says a death should be registered within 30 days. Begin promptly, because hospital, police, burial and family evidence is easier to assemble while events are recent.

Zimbabwe death registration is handled by the Civil Registry Department at Central Registry and its provincial, district and sub-office network. The Department specifically states that death-registration services are available through Central Registry, provincial and district offices and sub-offices. In most cases, begin with the office serving the place of death or burial and confirm that it can issue the certificate.

Death-certificate delays are especially difficult because families often need the document for an estate, pension, insurance claim or property transfer. Most delays arise when the death itself is not fully documented, the person applying has not proved their relationship or authority, or the facts supplied by medical, family and local records do not match.
